Breton Ability Centre in collaboration with the Nova Scotia Department of Community Services, is partnering to offer a Person-Directed Planning Program to assist individuals with disabilities/mental health challenges and their families.
Through the use of a Person-Directed Approach, individuals are given the opportunity to realize their strengths, talents, and dreams. Together we are hoping to facilitate the engagement of individuals in new opportunities for the future by connecting them and their supports with community connections and services.
The goal of the Person-Directed Planning Program is to build on an individual's dreams, strengths, and capabilities. This is done through the focus on development of relationships, as well as on ways an individual can access community resources to support a good life. Understanding what is of value and how an individual can participate in is/her community is key to the Person-Directed Planning process.
